Although volumetry remains the gold standard for diagnosing limb lymphedema, it cannot differentiate between lymphatic edema and adipose hypertrophy. Bioimpedance analysis (BIA) offers a more comprehensive approach to assessing body composition. However, the impact of complex rehabilitation programs on multi-compartment body composition in postmastectomy upper limb lymphedema has not been sufficiently studied.</p>&#13;
<p><bold>AIM. </bold>To investigate the effect of a comprehensive medical rehabilitation program, including laser therapy, magnetic therapy, exercise therapy, intermittent pneumatic compression, and cyclic trainer exercises, on body composition parameters assessed by BIA in patients with upper limb lymphedema, associated with radical mastectomy.</p>&#13;
<p><bold>MATERIALS AND METHODS.</bold> A cross-sectional study included 30 women aged 35–75 years with post-mastectomy lymphedema. Over 10 days, patients underwent comprehensive rehabilitation alongside a high-protein diet. The effectiveness assessment included anthropometry (circumferences of upper limb segments) and BIA of body composition before and after treatment.</p>&#13;
<p><bold>RESULTS.</bold> A statistically significant reduction was observed in the circumferences of all upper limb segments on both arms (<italic>p</italic> &lt; 0.05), except for the right elbow circumference. The most pronounced changes were noted in the proximal segments. BIA revealed a significant decrease in total (<italic>p</italic> = 0.00035), extracellular (<italic>p</italic> = 0.00008), and intracellular fluid (<italic>p</italic> = 0.000002), as well as a reduction in waist (<italic>p</italic> = 0.00004) and hip circumference (<italic>p</italic> = 0.0003). Positive changes in lean body mass (<italic>p</italic> = 0.0018) and musculoskeletal mass (<italic>p</italic> = 0.004), along with an improvement in basal metabolic rate (<italic>p</italic> = 0.026), were observed.</p>&#13;
<p><bold>DISCUSSION. </bold>The results demonstrate the effectiveness of the comprehensive approach in reducing lymphedema and normalizing fluid homeostasis. The pronounced effect in the proximal segments is consistent with literature data on the importance of restoring proximal lymphatic flow. The observed positive dynamics of changes in skeletal muscle mass (<italic>p</italic> = 0.004), active cell mass (<italic>p</italic> = 0.0018) and basal metabolic rate (<italic>p</italic> = 0.026) may indicate an increase in the level of metabolism in skeletal muscles due to an increase in the overall level of physical activity after the rehabilitation course.</p>&#13;
<p><bold>CONCLUSION. </bold>The comprehensive medical rehabilitation program is an effective means of correcting not only local manifestations of lymphedema but also systemic body composition disorders. The combined use of BIA and traditional anthropometry provides an objective multi-component assessment of rehabilitation effectiveness.</p></abstract><trans-abstract xml:lang="ru"><p><bold>ВВЕДЕНИЕ. </bold>Лимфедема верхних конечностей является частым и инвалидизирующим осложнением после радикального лечения рака молочной железы (РМЖ), с распространенностью 20–40 %. Несмотря на то что «золотым стандартом» диагностики лимфедемы конечностей остается волюметрия, она не позволяет дифференцировать лимфатический отек от жировой гипертрофии. Биоимпедансометрия (БИМ) предлагает более комплексный подход к оценке состава тела, однако влияние комплексных реабилитационных программ при постмастэктомической лимфедеме верхних конечностей на многокомпонентный состав тела изучено недостаточно.</p>&#13;
<p><bold>ЦЕЛЬ.</bold> Изучить влияние комплексной программы медицинской реабилитации, включающей лазеротерапию, магнитотерапию, лечебную физкультуру, переменную пневмокомпрессию и занятия на циклических тренажерах, на показатели состава тела по данным БИМ у пациентов с лимфедемой верхних конечностей, связанной с радикальным лечением РМЖ.</p>&#13;
<p><bold>МАТЕРИАЛЫ И МЕТОДЫ.</bold> В одномоментном исследовании участвовало 30 женщин в возрасте 35–75 лет с вторичной лимфедемой верхней конечности после мастэктомии по поводу РМЖ. В течение 12 дней пациентки проходили комплексную реабилитацию в условиях стационара центра. Оценка эффективности включала антропометрию (окружности сегментов верхних конечностей) и БИМ состава тела до лечения и после него.</p>&#13;
<p><bold>РЕЗУЛЬТАТЫ.</bold> После проведенного курса реабилитации отмечено статистически значимое уменьшение окружности всех сегментов верхних конечностей с отеком (<italic>p</italic> &lt; 0,05), за исключением окружности локтя руки справа. Наиболее выраженная динамика наблюдалась в проксимальных отделах. По данным БИМ, выявлено достоверное снижение общей (<italic>p</italic> = 0,00035), внеклеточной (<italic>p</italic> = 0,00008) и внутриклеточной жидкости (<italic>p</italic> = 0,000002), а также уменьшение окружности талии (<italic>p</italic> = 0,00004) и бедер (<italic>p</italic> = 0,0003). Отмечена положительная динамика показателей активной клеточной массы (<italic>p</italic> = 0,0018) и скелетно-мышечной массы (<italic>p</italic> = 0,004), улучшение основного обмена (<italic>p</italic> = 0,026).</p>&#13;
<p><bold>ОБСУЖДЕНИЕ.</bold> Результаты демонстрируют эффективность комплексного подхода в уменьшении лимфатического отека и нормализации жидкостного гомеостаза. Выраженный эффект в проксимальных отделах пораженной конечности согласуется с данными литературы о важности восстановления проксимального лимфооттока. Отмеченная положительная динамика изменения показателей скелетно-мышечной массы (<italic>p</italic> = 0,004), активной клеточной массы (<italic>p</italic> = 0,0018) и основного обмена (<italic>p</italic> = 0,026) может свидетельствовать о повышении уровня метаболизма в скелетных мышцах за счет повышения общего уровня физической активности после курса реабилитации.</p>&#13;
<p>ЗАКЛЮЧЕНИЕ. Комплексная программа медицинской реабилитации является эффективным средством коррекции не только локальных проявлений лимфедемы верхних конечностей, но и системных нарушений состава тела. Сочетанное использование БИМ и традиционной антропометрии обеспечивает объективную многокомпонентную оценку эффективности реабилитации.</p></trans-abstract><kwd-group xml:lang="en"><kwd>body composition</kwd><kwd>medical rehabilitation</kwd><kwd>chronic lymphedema</kwd><kwd>pneumatic compression</kwd><kwd>mastectomy</kwd><kwd>breast cancer</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>состав тела</kwd><kwd>медицинская реабилитация</kwd><kwd>хронический лимфатический отек</kwd><kwd>пневмокомпрессия</kwd><kwd>мастэктомия</kwd><kwd>рак молочной железы</kwd></kwd-group><funding-group><award-group><funding-source><institution-wrap><institution xml:lang="ru">Министерство здравоохранения Российской Федерации</institution></institution-wrap><institution-wrap><institution xml:lang="en">Ministry of Health of the Russian Federation</institution></institution-wrap></funding-source><award-id>056-03-2024-119</award-id></award-group></funding-group></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>DiSipio T., Rye S., Newman B., Hayes S.
