However, to date, it remains unknown to what extent the actually prescribed procedures of the individual medical rehabilitation program correspond to the stated rehabilitation goal, as well as what proportion of "non-target" prescriptions are made by different specialists at the second stage of rehabilitation.</p> <p><bold>AIM. </bold>To examine the relationship between the goal formulated by the multidisciplinary medical rehabilitation team and the procedures prescribed within the Individual Medical Rehabilitation Program (IMRP) in patients with central nervous system disorders at the second stage of medical rehabilitation, and to determine the frequency and structure of prescriptions not related to the rehabilitation goal, stratified by each multidisciplinary rehabilitation team (MDRT) specialist.</p> <p><bold>MATERIALS AND METHODS. </bold>The retrospective study included 312 patients who completed a rehabilitation course in a department for patients with central nervous system disorders. The following were analyzed: all documented rehabilitation goals (<italic>n</italic> = 312); each procedure of the IMRP (total of 2,415 prescriptions) with an assessment of its relationship to the goal (related/unrelated); the proportion of goal-unrelated prescriptions, separately for the physical rehabilitation specialist, ergorehabilitation specialist, medical speech-language pathologist, medical psychologist, and physical and rehabilitation medicine (PRM) physician.</p> <p><bold>RESULTS. </bold>Of all prescribed IMRP interventions, 54.3 % were directly related to the established rehabilitation goal, whereas 45.7 % showed no such relationship. The physical rehabilitation specialist delivered goal-related interventions in 82.3 % of cases. The proportion of goal-oriented prescriptions was considerably lower among other specialists: occupational therapy specialist — 26.1 %, PRM physician — 26.2 %, medical psychologist — 25.6 %, and medical speech-language specialist — 20.7 %. Consequently, between 73.9 % and 79.3 % of prescriptions from these specialists lacked documented justification by the stated rehabilitation goal.</p> <p><bold>DISCUSSION. </bold>The predominance of mobility-related goals (walking, 86.9 %) creates documented grounds for a high proportion of goal-unrelated prescriptions among specialists whose professional scope extends beyond mobility (speech therapist, medical psychologist, occupation therapist). The low proportion of goal-oriented interventions in these specialists (20.7–26.1 %) reflects not a lack of competence, but rather the narrowness of goal formulation that fails to incorporate communication, cognitive, and daily-living needs. Thus, technically sound SMART goal-setting without multidisciplinary consensus does not guarantee content validity of the rehabilitation plan.</p> <p><bold>CONCLUSION. </bold>At the second stage of medical rehabilitation for patients with central nervous system disorders, a substantial disconnect has been identified between goal setting and the actual intervention program (IMRP). For several multidisciplinary rehabilitation team specialists — specifically, the medical speech-language pathologist, medical psychologist, and occupational therapy (ergorehabilitation) specialist — nearly half of all prescribed interventions are unrelated to the stated goal. These findings support the need to develop and test standardized goal-setting tools for routine clinical practice in neurorehabilitation within prospective studies.</p></abstract><trans-abstract xml:lang="ru"><p><bold>ВВЕДЕНИЕ.</bold> В основе эффективной медицинской реабилитации лежит персонализированное целеполагание, определяющее логику и объем вмешательств мультидисциплинарной реабилитационной команды (МДРК). Однако до настоящего времени оставалось неизученным, насколько фактически назначаемые процедуры индивидуальной программы медицинской реабилитации (ИПМР) соответствуют заявленной реабилитационной цели, а также какова доля «нецелевых» назначений у каждого из специалистов на втором этапе реабилитации.</p> <p><bold>ЦЕЛЬ ИССЛЕДОВАНИЯ. </bold>Изучение взаимосвязи между целью, сформулированной мультидисциплинарной командой специалистов по медицинской реабилитации, и процедурами, включенными в процедуры ИПМР, у пациентов с заболеваниями центральной нервной системы (ЦНС) на втором этапе медицинской реабилитации, а также выявление частоты и структуры назначений, не связанных с целью медицинской реабилитации по отношению к каждому из специалистов МДРК.</p> <p><bold>МАТЕРИАЛЫ И МЕТОДЫ.</bold> В ретроспективное одноцентровое исследование включено 312 пациентов, завершивших курс реабилитации в отделении медицинской реабилитации пациентов с нарушением функции ЦНС № 1 ФГБУ «Федеральный центр мозга и нейротехнологий» ФМБА России с 22.04.2025 по 31.12.2025. Анализировались: все сформулированные реабилитационные цели (312); каждая процедура ИПМР (всего 2415 назначений) с оценкой ее связи с целью (связана/не связана); доля назначений, не связанных с целью, отдельно для специалиста по физической реабилитации, специалиста по эргореабилитации, медицинского логопеда, медицинского психолога и врача физической и реабилитационной медицины (ФРМ).</p> <p><bold>РЕЗУЛЬТАТЫ. </bold>Прямо связано с поставленной целью было 54,3 % от всех назначенных процедур ИПМР; 45,7 % процедур такой связи не имели. Специалист по физической реабилитации назначал процедуры, связанные с целью, в 82,3 % случаев. У остальных специалистов доля целеориентированных назначений оказалась ниже: у специалиста по эргореабилитации — 26,1 %, у врача ФРМ — 26,2 %, у медицинского психолога — 25,6 %, у медицинского логопеда — 20,7 %. Таким образом, от 73,9 % до 79,3 % назначений этих специалистов не были документально обоснованы поставленной реабилитационной целью.</p> <p><bold>ОБСУЖДЕНИЕ. </bold>Установлено, что доминирование цели в двигательном домене (восстановление ходьбы; 86,9 %) создает документальные предпосылки для высокого процента «нецелевых» назначений у специалистов, чьи профессиональные задачи выходят за рамки мобильности (медицинский логопед, медицинский психолог, специалист по эргореабилитации). Низкая доля целеориентированных процедур у этих специалистов (20,7–26,1 %) отражает не столько недостаток их компетенций, сколько неполноту самого целеполагания, которое не охватывает коммуникативные, когнитивные и социально-бытовые потребности пациента. Таким образом, технически корректное SMART-целеполагание без междисциплинарного согласования не гарантирует содержательной обоснованности реабилитационной программы.</p> <p><bold>ЗАКЛЮЧЕНИЕ. </bold>На втором этапе медицинской реабилитации пациентов с заболеваниями ЦНС выявлен значительный разрыв между целеполаганием и реальной программой вмешательств (ИПМР): у ряда специалистов МДРК (медицинский логопед, медицинский психолог, специалист по эргореабилитации) почти каждое второе назначение не связано с заявленной целью, а полученные данные обосновывают необходимость разработки и апробации стандартизированных инструментов целеполагания для реальной клинической практики медицинской реабилитации по профилю нейрореабилитации в проспективных исследованиях.</p></trans-abstract><kwd-group xml:lang="en"><kwd>rehabilitation goal-setting</kwd><kwd>medical rehabilitation program</kwd><kwd>medical rehabilitation</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>целеполагание в реабилитации</kwd><kwd>программа медицинской реабилитации</kwd><kwd>медицинская реабилитация</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Levack W.M., Dean S.G., Siegert R.J., McPherson K.M.
