Клеточная терапия болезни периферических артерий

РЕЗЮМЕ
Болезнь периферических артерий способствует снижению толерантности к физической нагрузке, нарушениюпроприоцептивной чувствительности, мышечной атрофии и слабости, в поздних стадиях приводит к критическойишемии конечностей, изъязвлению и гангрене. Современные методы лечения путем восстановления кровоснабжения конечности, ограничены и часто имеют плохие исходы вследствие многоуровневой артериальной болезни. Терапевтический ангиогенез считается методом лечения больных с критической ишемией конечностей, которые не соответствуют критериям хирургической реваскуляризации. Ангиогенез может быть индуцирован факторами роста и/или препаратами стволовых клеток костного мозга, содержащими мезенхимальные стромальные клетки и эндотелиальные клетки-предшественники, способные образовывать сосудистый эндотелий. Многочисленные исследования,опубликованные в 2000–2014, показали, что стволовые клетки костного мозга, содержащие мезенхимальные стволовые клетки и эндотелиальные клетки-предшественники, были успешно и безопасно изолированы и имплантированыпациентам с критической ишемией конечностей путем внутривенных или внутримышечных инфузий и привели кзначительному клиническому улучшению у пациентов по шкале боли, частоте ампутации, расстоянию возникновения хромоты и ангиографическим признакам. Эти исследования показали значительное удлинение безрецидивногопериода, определяемого как удвоение изъязвления, большая ампутация или новая гангрена. Это исследования недемонстрируют никаких негативных последствий в связи с внутривенной или внутримышечной инъекцией стволовых клеток аутологичного костного мозга. У пациентов, получивших трансплантацию, наблюдались улучшение микроциркуляции и полное заживление язв. После применения стволовых клеток удалось избежать ампутации стопы.
