Физические тренировки как способ коррекции дисфункции мышечно-венозной помпы голени у пациентов с посттромботической болезнью: обзор

РЕЗЮМЕ

ВВЕДЕНИЕ. Дисфункция мышечно-венозной помпы (МВП) голени при венозной обструкции приводит к неэффективному опорожнению венозной крови из дистальных отделов нижних конечностей и может рассматриваться как терапевтическая мишень при лечении хронических заболеваний вен, а физические упражнения (ФУ) могут быть эффективным методом повышения гемодинамической эффективности МВП.

ЦЕЛЬ. Поиск и аналитический обзор опубликованных в международных базах данных результатов научных исследований, посвященных изучению роли дисфункции МВП в патогенезе хронической венозной недостаточности (ХВН), а также эффективности применения различных видов физических тренировок.

МАТЕРИАЛЫ И МЕТОДЫ. В базах данных eLIBRARY.RU, Scopus, PubMed, Web of Science с 1998 по 2025 г. был отобран 51 источник, из которых 19 составили систематические обзоры, включающие 2 Кокрановских обзора, 5 — практические руководства, а также 27 — рандомизированные контролируемые исследования (РКИ).

РЕЗУЛЬТАТЫ. Установлено, что у пациентов с легкой степенью ХВН применение ФУ приводит к улучшению показателей венозного рефлюкса, мышечной силы, диапазона движений в голеностопном суставе (AROM — Ankle Range of Motion) и качества жизни (КЖ). У пациентов с тяжелой ХВН ФУ увеличивали фракцию выброса, снижали фракцию остаточного объема и улучшали мышечную силу, AROM. Показано, что изотонические упражнения, повышая мышечную выносливость, эффективность и силу икроножных мышц, улучшают их сократительную способность и общую гемодинамику в конечностях с венозными трофическими язвами (ВТЯ). Установлено, что эффективность МВП также зависит от подвижности голеностопного сустава и силы сокращения икроножных мышц.

ОБСУЖДЕНИЕ. При наличии убедительной доказательной базы для назначения пациентам с ВТЯ прогрессивных силовых нагрузок и аэробной активности отсутствуют исследования, изучающие эффективность изокинетических тренировок в комплексной реабилитации пациентов с посттромботической болезнью (ПТБ). Это подчеркивает необходимость разработки современных протоколов изокинетических тренировок у пациентов с ПТБ, поскольку при этом достигается улучшенная производительность за счет максимальной нагрузки на мышцы, улучшающей двигательное обучение за счет обратной связи в режиме реального времени.

ЗАКЛЮЧЕНИЕ. Изучение влияния изокинетических физических тренировок на функцию икроножной помпы, мышечную силу, AROM и качество жизни пациентов с ПТБ является перспективным направлением медицинской реабилитации.
